Utah Cardiology uses a kiosk check-in system that streamlines your check-in process. Please review some information about this system here: Kiosk Check-in Process. If we have an email address on file, you will receive a reminder 1-2 days in advance of your appointment. When you arrive at our office, please have your driver’s license and insurance cards available.
The following are some forms that aren’t addressed at the kiosk.

If you are a new patient to our clinic or haven’t been seen in our clinic for over one year, please review the following:
- Please bring a completed NEW Patient History Form
- Please review the Notice of Privacy Practices
